ABSTRACT

Objective To evaluate and compare the diagnostic efficiency of X-ray mammography and breast ultrasound for suspected breast lesions. Methods A total of 80 female patients with suspected breast lesions diagnosed by X-ray mammography or breast ultrasound were selected as the study subjects. The histopathological testresults served as the gold standard, and the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of X-ray mammography and breast ultrasound for the diagnosis of breast lesions were estimated and compared. Results Among the 80 female patients with suspected breast lesions, 57 patients were histopathologically diagnosed with breast cancer and 23 patients with benign breast lesions. The sensitivity, specificity, NPV, PPV, and accuracy of X-ray mammography for the diagnosis of suspected breast lesions in women were 63.16%, 73.91%, 44.74%, 85.71%, and 66.25%, respectively, and those of breast ultrasound were 78.95%, 21.74%, 29.41%, 71.43%, and 62.50%, respectively. There were no significant differences between X-ray mammography and breast ultrasound for the diagnosis of suspected breast lesions in women in terms of sensitivity (χ2 = 3.45, P > 0.05), NPV (χ2 = 1.15, P > 0.05), PPV (χ2 = 2.92, P > 0.05), and accuracy (χ2 = 0.25, P > 0.05). However, specificity showed a significant difference between X-ray mammography and breast ultrasound (χ2 = 12.55, P < 0.01). Conclusion A comparable accuracy of X-ray mammography and breast ultrasound is found for the diagnosis of suspected breast lesions in women. However, X-ray mammography presents a significantly higher specificity than breast ultrasound for the diagnosis of suspected breast lesions in women.

ABSTRACT

Lung cancer is a malignant tumor with high incidence and mortality in the world, which seriously threatens people's safety and health. Early diagnosis of lung cancer is the key part in the process of prevention and treatment of lung cancer. It can improve the survival of patients with lung cancer. Exosomes are closely related to the invasion and metastasis process of tumor, it plays an important role in the development of lung cancer. Biomarkers based on exosomes have become a powerful diagnostic tool of lung cancer. Exosomes are lipid bilayer vesicles with uniform size and diameter of 30 nm-200 nm secreted by cells. Exosomes contain different types of nucleic acids and proteins. These nucleic acids and proteins are derived from their parent cells (including parent cancer cells), which have a wide range of physiological functions, including immune regulation, intercellular communication and other physiological activities. Biomacromolecules in exosomes, such as single-stranded RNA, long noncoding RNA, microRNA, protein and lipids, which can provide valuable genetic information for early clinical diagnosis of lung cancer. This review described the origin, structural characteristics, extraction methods, biological characteristics of exosomes and the relationships of exosomes in the early diagnosis of lung cancer.

ABSTRACT

OBJECTIVE:To pr ovide refe rence for the future research on drug safety legislation and scientific law enforcement and justice. METHODS :The journal articles published from Jan. 1st,1985 to Dec. 31st,2019 in CNKI were searched by using “subject=drug crime ,food and drug crime or drug and criminal law ”as retrieval words but without “subject=drug”. By screening literature, with the emphasis on the content ,with the help of CiteSpace 5.6.R4 software, the legislative dynamics of drug safety and typical drug injury events in China were sorted out and analyzed. The development of the research on drug safety crimes in China were analyzed from four aspects :research output ,research academic groups ,research discipline perspective and research focus. From the aspects of criminal policy ,criminal legislation ,major drug injury cases ,administrative supervision concept and administrative legislation ,the factors influencing the research changes in each stage were combed and excavated ;on this basis ,the relevant researches on drug safety crime were reviewed and suggestions were put forward for its improvement path. RESULTS & CONCLUSIONS:A total of 852 literatures were retrieved ,and 473 valid literatures were included. The research on crimes against drug safety in China has gone through the exploration period (1985-1996),transition period (1997-2010)and vigorous period (2011-2019). The number of achievements has been increasing ,and the scale of academic groups has continued to expand , forming a discipline distribution with criminal law research as the main research ,public security science research as the auxiliary , and natural science research as the foreign aid ,focusing on the issues of legislation ,judicature and criminal law value ,for example, various academic researches was conducted with major hazardous drug cases as the breakthrough point. Among the influential factors of the researches on drug safety crime ,criminal policy and legislation ,administrative legislation and regulatory evolution were the internal logic ,and the sudden drug injury cases were the important external factors. For this purpose ,to promote theoretical innovation and practical exploration of drug safe ty crime research ,research on drug safety crime in the future should focus on the whole chain of drug safety , using method and empirical research methods.

ABSTRACT

Objective To study the calculation of the cost consumption index adjusted with major diagnostic classification (MDC), for optimal use of DRGs index in hospital performance evaluation. Methods The adjustment method of CMI value in DRGs index system was used as reference, and we compared the different cost consumption indexes(both MDC adjusted and non-adjusted) of two hospitals(S and Y) in Guangdong province. Then we compared the different rankings of 82 tertiary general hospitals in the province before and after the adjustment. Results The cost consumption index of S hospital was higher than that of Y hospital by the non-adjusted method (1.30 >1.28). But as calculated by the adjusted method, the index of S hospital was significantly lower than Y hospital (1.31 <1.38). The rankings of these 82 hospitals also showed major changes, which prove that the cost consumption index, the same as CMI,will be affected by MDC cases makeup. Conclusions The MDC adjusted cost consumption index, when applied to hospital performance evaluation, renders more stable and reasonable results. This is an evidence that the adjusted cost consumption index is of great practical value in hospital performance evaluation.

ABSTRACT

In order to solve the problem of selection and in vivo delivery problem in siRNA treatment, hepatitis B virus (HBV) HBx gene which could be targeted by siRNA was studied. The siRNA expression plasmid which specific inhibits HBx expression was obtained by in vitro selection via a dual-luciferase plasmid including HBx-Fluc fusion protein expression domain. The selected siRNA expression plasmid was then encapsulated in PEG-modified cationic liposome, which was devoted into pharmacodynamic studies at both cellular and animal level. The results illustrated that the cationic liposome which encapsulated siRNA expression plasmid could effectively inhibit HBx gene expression both in vitro and in vivo.
